INSTRUCTIONS FOR COMPLETING CONSULTATION QUESTIONNAIRE
In order to schedule a telephonic or in-person consultation with Cella & Associates, LLC., the following questionnaire must be completed in its entirety and submitted to us by clicking the “submit” button at the end of this questionnaire. If you choose, you may send us the completed questionnaire by fax at 973-473-6899.

After submitting the questionnaire, you should telephone us at 973-473-6889 to schedule a consultation, specifying whether you prefer telephonic or in-person. You should also have your credit card number and expiration date available to pay the consultation fee. (Visa, MasterCard, Discovery).

Our consultation fee is $100.00 (U.S.D.), which includes our review and analysis of the information provided on the questionnaire, and a thirty-minute consultation. If retained, on the day of the consultation, the consultation fee will be credited toward the legal fees. It should be noted that having a consultation with us, without retaining us, does not establish an attorney-client relationship. Accordingly, unless and until retained, Cella & Associates, LLC will have no obligation to perform any legal services on your behalf. If, after submitting the questionnaire, you fail to schedule an appointment, or if after the consultation, you do not retain this firm, the completed questionnaire will be disposed of.

On the day and time of your scheduled consultation, please call the office at the number provided to you when you scheduled your consultation. Should you call late, your consultation time will be decreased accordingly.

Additionally, please be advised that any information provided to us in your questionnaire will be kept strictly confidential in accordance with “attorney/client privilege”.

Finally, when completing the questionnaire, keep in mind that the information requested pertains to the person seeking immigration services. For example, if you are a spouse or employer or potential employer, the information provided pertains to the alien or aliens for whom an immigration benefit is sought.
